In the concept of Tactical Combat Casualty Care (TCCC), the use of the MARCH algorithm is recommended during the Tactical Field Care stage. This stage takes place in the yellow or green zone when the threat of enemy fire is no longer present, and you can focus on providing comprehensive assistance to the wounded. The MARCH algorithm defines priorities and the sequence of actions for providing aid to the wounded.

У концепції тактичної бойової допомоги пораненим на етапі тактичної польової допомоги рекомендується використання алгоритму MARCH. Цей етап відбувається в жовтій або зеленій зоні, коли загроза вогню противника вже відсутня, і можна зосередитися на наданні комплексної допомоги пораненим. Алгоритм MARCH визначає пріоритети та послідовність дій для надання допомоги пораненим.

M – MASSIVE HEMORRHAGE

✔️Assess for unrecognized hemorrhage and control all life-threatening bleeding. – Оцініть наявність прихованої кровотечі та контролюйте кровотечі, що загрожують життю.

💪Use one or more tourniquets if necessary. – За потреби застосуйте один або кілька джгутів.

🩹Use a hemostatic dressing for compressible hemorrhage not amenable to tourniquet use. – Використовуйте гемостатичну пов’язку для кровотечі, яка не піддається накладанню джгута.

A – AIRWAY MANAGEMENT

Casualty with airway obstruction or impending airway obstruction: (1) allow a conscious casualty to assume any position that best protects the airway, to include sitting up, (2) сhin lift or jaw thrust maneuver, (3) nasopharyngeal airway. (4) place an unconscious casualty in the recovery position.

R – RESPIRATION

In a casualty with progressive respiratory distress and known or suspected torso trauma, consider a tension pneumothorax: decompress the chest on the side of the injury at the primary or alternate site.

C – CIRCULATION

If not in shock PO fluids are permissible if casualty is conscious and can swallow. If in shock resuscitate with whole blood (preferred).

💡Preferred – бажаний

H – HYPOTHERMIA

Minimize casualty environmental exposure and promote heat retention. Keep personal protective gear on if feasible. Replace wet clothing if possible.
