The FAST (Focused Assessment with Sonography for Trauma) protocol is a rapid bedside ultrasound examination used primarily in emergency and trauma settings to quickly assess patients for internal bleeding or fluid in the abdominal and thoracic cavities. It’s widely used to evaluate patients with blunt abdominal trauma, where quick detection of bleeding can be critical for timely intervention.
FAST (Фокусована оцінка за допомогою сонографії при травмах) — це швидке ультразвукове обстеження, яке проводять безпосередньо біля ліжка пацієнта, в основному в екстрених і травматологічних відділеннях, щоб швидко оцінити наявність внутрішньої кровотечі або рідини в черевній та грудній порожнинах. Його широко використовують для обстеження пацієнтів із тупими травмами живота, коли швидке виявлення кровотечі може бути критичним для своєчасного втручання.

🗣️FAST/e-FAST protocol is designed to provide a simple “yes or no” answer regarding the presence of bleeding. It is not intended to quantify the bleeding nor evaluate organ lesions due to its limited accuracy for these purposes. Moreover, the amount of bleeding and/or the identification of organ lesions will not change patient’s management.

🗣️The FAST protocol is done mainly with B-mode images using a convex, low-frequency probe. Four regions will be addressed in the traditional FAST protocol: pericardium; right upper abdominal quadrant; left upper abdominal quadrant; and pelvis. e-FAST protocol will also include a 5th region: the pleural spaces. Usually, the patient will be in the supine position. If possible, raising or spreading the patient’s arms may help evaluate the upper abdominal quadrants.

There are three classical indications:

🗣️FAST/e-FAST has achieved a widespread utilization worldwide over the past decades and revolutionized trauma care, reducing unnecessary laparotomies, and saving precious time in life-threatening trauma situations. Despite CT is considered the gold-standard for traumatic lesions’ investigation, FAST is more accessible, quicker, cheaper, and with less risks, contributing significantly to trauma care and decision-making algorithms.
